Getting Hospice in place in a timely manner will provide support for both the patient and the family

You are able to select the Hospice organization that you would prefer.
Services can be provided to you in your private residence, community assisted living, adult care home or
nursing facility. Hospice not only works with the patient, but with the entire family as well.
Integrated Care Consultants will assist you to understand the Medicare Hospice benefit and access
support for the patient, family and chose friends who will be affected by this process. Hospice service
support will extend to the family after the loved one has passed.
